Directive to Physicians (Living Will) A Living Will usually covers specific directives as to the course of treatment to be taken by caregivers or, in some cases forbidding treatment, even food and water. This document will usually cover your desires concerning life-sustaining treatment, terminal conditions and irreversible conditions. HIPAA Form (Health Insurance Portability & Accountability Act)- An authorization for disclosure of confidential health care information for those you trust.
